Chiari CARE Second Opinion / Remote Consultation

Thank you for choosing Chiari CARE at Weill Cornell Medicine for your remote consultation. This service can be used for either an initial evaluation or a second opinion after receiving your diagnosis. Although in-person evaluations are always best, we understand travel can often be prohibitive, particularly for those not in the Northeastern United States, which is why we offer an option for those seeking consultations from other parts of the country and internationally.

How it works:

  • Specify your diagnosis
  • Write an overview of your symptoms
  • Upload your imaging—including brain, cervical, thoracic, and lumbar MRIs
  • Upload MRI Reports
  • Upload all other relevant documents:
  • Neurology Office notes
  • Neurosurgeon Office notes including prior surgical reports
  • Ophthalmology reports
  • Geneticist or Cardiologist reports confirming EDS or POTS
  • Sleep Study Reports
  • Swallow Study Reports
    • There is a $300 fee for this service, which you may pay directly through the site by credit card.
    • You will receive an email confirming that your account has been established and that your information is waiting to be reviewed.
    • In four to six weeks you will receive a thorough written evaluation from Dr. Jeffrey Greenfield with a diagnosis, surgical recommendations, and any additional suggested testing or imaging. These suggestions should be shared with your local internist, pediatrician, neurologist, or neurosurgeon to order the appropriate exams.
  • If you choose to follow any of the recommendations in the remote consultation, and wish to follow-up with Dr. Greenfield, the next steps would be to contact the Chiari CARE coordinator at Weill Cornell Medicine and address with us the options for continuation of care. The remote consultation is not part of the formal medical record and does not assume ongoing consultative discussion with the Chiari CARE team.
