Chiari CARE Second Opinion Program

Thank you for choosing Chiari CARE at Weill Cornell Medicine for your second opinion. In addition to in-person appointments, we offer two other options for those seeking second opinion evaluations. Please review these options, which will allow you to receive the most timely and informative evaluation based on your medical history.

Please be aware that online and phone evaluations should not be considered a substitution for an in-person evaluation by a physician. A provider cannot always fully assess and diagnose a patient through an online portal or phone conversation, since important information is obtained through physical assessments and in-person discussions. Online and phone evaluations should only be a first step to obtaining a second opinion from a neurosurgeon. If you wish to follow up with the suggested recommendations or discuss your case in more detail, you may want to schedule an appointment. Option 1: Online Second Opinion
This option is most beneficial for, but not limited to, those who have a diagnosis of Chiari malformation or one of the related etiologies but who have not received a prior surgical intervention. To request an online review, you’ll begin by uploading your imaging—including brain, cervical, thoracic, and lumbar MRIs—to a secure online account. There is an additional option to upload other data, including radiographic reports and/or office notes that pertain to your Chiari care from your PCP, neurologist, or neurosurgeon. You may also upload other reports that pertain to your medical history, including sleep studies, ophthalmology consults, or office notes from a geneticist or cardiologist confirming an EDS or POTS diagnosis. There is a $300 fee for this service, and you’ll receive a written evaluation of your case. Click here to request an online second opinion evaluation.

Please note that your records may be reviewed by either 
Dr. Greenfield or Dr. Hoffman. If you prefer a specific surgeon for your review, please specify the name in the “diagnosis/ symptoms” section when uploading your records

Option 2: Second Opinion Phone Call
This option is most beneficial for, but not limited to, individuals with a diagnosis of Chiari malformation or related etiologies and who haveundergone a previous surgical intervention. Our neurosurgeons prefer the phone option for postsurgical individuals since these are often more complex discussions; a phone call allows them to more thoroughly discuss your medical history and prior care. To arrange for a second opinion phone call, please start bysending all the above-mentioned records (plus an operative note from your previous surgery) to our office. Once your records have been reviewed, we will contact you to set up your telephone appointment. If you have any questions, please call 212-746-2363.

Since the Weill Cornell Brain and Spine Center introduced Chiari CARE, our patient volume has increased substantially. For those seeking an online or telephone second opinion evaluation with Dr. Jeffrey Greenfield, note that it may take at least two weeks for you to be contacted for an initial intake. If you believe a response is time-sensitive, you may want one of our other neurosurgeons within Chiari CARE to provide your second opinion.
